For linking to different language versions of the current page, I made these hyperlinks:

<a href="/nl/pageid" lang="nl-nl" hreflang="nl-nl">Nederlands</a>
<a href="/en/pageid" lang="en-gb" hreflang="en-gb">English</a>
  1. Should either or should both lang= and hreflang= be used?
  2. What are the benefits of en over en-gb in such hyperlinks?
  3. What are the benefits of en-gb over en in such hyperlinks?

  1. I would recommend you to use both lang and hreflang because they will improve your visibility overhaul, as some search engines will target hreflang

And in fact, it’s the same for the en vs en-gb choice. Not so different, but some search engine love them better, so use en-gb. It doesn’t hurt.
